Initial Agency Briefing

Thursdays · 11-12 or Fridays · 8-9

  • Supervisors and workers meet with Karen to identify:

    Goal of the FPM

    Gather names/relations/contact info for desired participants

    Agency bottom lines

  • Written report provided that can be entered in OASIS

FPM Prep

Evenings and weekends

  • All identified participant contacted

  • FPM process and meeting purpose thoroughly explained

  • Assist participant in identifying their role and what resources they may offer before they enter the room

  • Provide Zoom education, instructions and practice

  • Email Calendar Hold Dates with M.M.B Zoom link

FPM Meetings (2 hours)

Mondays & Tuesdays · 5-8:30 pm

  • M.M.B Zoom opens 15 minutes prior to meeting start time

  • Meaningful and dynamic meeting facilitation is provided to move the group through the process

  • Ensures high-quality decision results from the meeting

  • Accurately record information and decisions

  • Final Report provided that can be entered in OASIS

Agency debrief

Thurdays · 11-12 or Fridays · 8-9

  • Supervisors, workers and facilitator meet to identify areas of success and opportunities for improvement.

  • Data that satisfies State reimbursements provided

Planning for children begins at the FIRST contact

Lifelong family connections are crucial for children and adults

When children cannot live safely with their families, the first consideration for placement will be with kin

Increasing Kinship and decreasing Congregate Care is a worthy mandate

  • At Removal or any time a child is at high risk of Out of Home Placement

  • Two weeks before placement disruptions or anytime identifying supports and family history is needed

  • Family Finding is helpful Before, During and After a crisis

  • Anytime it seems Congregate Care is the only option

  • Using provided Accurint, I will reach out to all potential kin, fictive kin or other sources of support

  • Urgent need of the child will be communicated and I will assess their ability and desire to be of service

  • Documentation provided that will satisfy the State requirement for Foster Care Relative Search in OASIS
